5a91893b86 test: drain the detached SubagentStart continuation before the bridge spec ends
The markers are touched mid-script, so runPoint's continuation chain (child
exit → merge → the listener's detached .then) can still be in flight when the
marker poll resolves; a vitest worker that exits first leaves the inject
condition's short-circuit path uncounted. Observed as a CI-only 99.03%
branch-coverage flake on hooks-claude — surfaced by this branch shifting suite
timing, latent before it. Two macrotask rounds pin the path deterministically.

4641f1a851 chore: absorb the no-nudge seam into the workflow tree
The parent branch dropped structuredNudgeRetries; the two workflow-side test
setups stop passing it, and the RFC's foundation paragraph now describes the
current design (final-ASSEMBLY enforcement logged via request/header, the
post-capture pre-execute deny, the start() schema snapshot, no re-prompt)
instead of the retired agent/request + nudge shape.

b907c20213 review: drop the structured-output nudge; FIXME the context-global registry constraint
Two human review directives:

- No re-prompt. A structured child that finishes a turn cleanly without
calling structured_output settles error to the parent immediately —
readResult already carried that mapping; the nudge loop only delayed it.
Deletes the loop, its cancellation-window guard, STRUCTURED_OUTPUT_NUDGE,
and the structuredNudgeRetries Config on both backends.

- FIXME in the structured module doc: per-agent/per-session tool registry and
prompt assembly would dissolve the final-assembly enforcement dance (the
placeholder tool, the swap, the strip, the global-registration lifetime).

a520965f09 fix review findings: post-capture tool calls denied; schema snapshotted at start
Two bot findings on the structured runtime:

- Terminal means terminal WITHIN the step: a model response listing
structured_output before further tool calls executed those calls after the
final answer was accepted (the turn-continuation veto only fires at step
end). A third runtime listener now denies every call for a captured agent
at the tools/pre-execute gate — dispatch skipped, isError result naming the
contract. Calls preceding the capture in the same response are untouched.

- The output schema is structuredClone'd before the subset assertion: the
caller keeps its reference, so asserting and attaching the original let a
post-start() mutation drift the enforced schema away from the asserted one.
The clone pins assertion, model-visible parameters, and validation to one
  value.

a4152705ef fix review finding: abort tests must prove the hook process is gone
Codex review of the draft: the abort-on-dispose tests asserted only that
dispose() resolved and no warning fired — a regression back to untracked
fire-and-forget runs would pass both. The hook script now records its
shell PID before sleeping, and after dispose the test asserts
process.kill(pid, 0) throws: the drain resolves only after the killed
run settled (the child reaped), so ESRCH-by-dispose-return is
deterministic under the correct implementation, while an alive or
unreaped process fails it. Verified the discriminator by gutting drain()
locally: exactly these two tests fail.

f0ef280848 fix: identity-check name resolutions across the pasted closure; exclude the catalog from i18n pairing
Review findings on the config catalog:

The transitive-paste closure deduplicated by bare name, so two DIFFERENT
package-local declarations sharing a name would silently collapse to the
first one pasted — and a reference satisfied by the wrong declaration is a
wrong catalog, not just a thin one. Every resolution is now identity-checked
by source pointer: reaching the same declaration again is benign, while a
name that resolves to two distinct declarations, to conflicting imports, or
to a declaration in one file and an import in another is a violation — a
verbatim fence has a single flat namespace, so the fix is a rename at the
source. A spec case pins the two-declarations collision.

docs/config-catalog.md also joins the translation-pairing exclusions: it is
a generated English-only doc, and without the entry the gate would accept a
hand-added .zh.md twin it must reject.

7a35afa6fe Merge branch 'structured-output-subagent-seam' into worktree-dynamic-workflows
Restack on the carved-out foundation (#192), per review feedback on #170.
The seam files resolve to the carve-out's revision — its prompt-order
neutrality fix (backends no longer inject 'tools'; the structured runtime
gates its own capture-tool registration) restores the subagent tools to
master's front position, so every recorded fixture is re-recorded on the
stacked tree and the authored error-finish/cancel headers re-patched to the
stacked tool list ([subagent, subagent_fork, workflow, todo_write, ...]).

1fd9e2a9ab Pin request-header content in one snapshot scenario
Every session.jsonl fixture embedded the full composed system prompt and
complete tool-schema list in its request/header event (~8 KB on one line,
identical across the suite), so any prompt or tool-schema edit forced a
re-record or hand-edit of every fixture — see the dynamic-workflows PR for
the churn pattern this removes.

Now exactly one scenario (text-turn, flagged pinsHeader) commits and
compares that content verbatim; every other fixture stores and compares it
as {{system}}/{{tools}} tokens via the new pure scrubRequestHeaders
normalizer (applied to both compare sides and to record-mode writes, so a
re-record cannot reintroduce the content). request/header-delta payloads
are scrubbed the same way; config/reason stay verbatim — a model swap
SHOULD churn every fixture, a prompt edit should not. Replay is unaffected:
script derivation reads only assistant/chunk events.

Fixture meta-guards enforce the split: non-pinning fixtures must be fixed
points of the scrub, the pinning fixture must not be, and exactly one
scenario pins. Committed fixtures migrated through the same function.

Docs: pinned-header RFC (implemented/testing), base snapshot RFC + testing
policy + llm-replay module doc/README updated.

74502fa8c2 Structured output on the subagent seam: schema subset, capture runtime, spawn/fork support
Carved out of #170 per review feedback — the foundation the workflow tool
builds on, now standing alone on master:

- dsh-tools: the structured-output JSON Schema subset (StructuredOutputSchema,
assertSupportedOutputSchema, validateStructuredValue) — rejects loud outside
the enforced subset, listing every violation
- dsh-subagent: SubagentStartRequest.outputSchema / SubagentResult.structured
become a real capability; the service rejects a schema'd request whose
  provider lacks it
- dsh-subagent-inprocess: the shared structured runtime — one global
structured_output capture tool, a prepend final-assembly listener that
strips the placeholder for plain agents and swaps in the run's own schema
(plus the calling instruction as a trailing section) for structured
children, an agent/turn-continuation veto once captured, and the
capture/nudge loop in the run driver (structuredNudgeRetries, cancellation
honored mid-nudge); lifetime refcounted by backends and live runs
- subagent-spawn / subagent-fork flip outputSchema: true

One deliberate divergence from the #170 revision: the backends do NOT add
'tools' to their plugin inject. Doing so deferred their apply past the todo
plugin, and the delegation tool mirrors provider lifecycle — so the
model-visible tool order of every existing prompt changed, invalidating every
recorded snapshot fixture. The runtime now gates its capture-tool registration
on tools availability itself (sync when live, a scoped inject fiber when the
Loader starts the backend first), keeping this PR byte-invisible to existing
transcripts: all 35 snapshot scenarios pass against master's fixtures
unchanged.

9ca31ab193 fix(hooks): drain detached hook runs on bridge dispose
The emit-shaped hook points (SessionStart, SubagentStart, SubagentStop)
run fire-and-forget: no seam awaits the run chain, so disposing a bridge
could strand a live hook process and let a late continuation inject into
a disposed context. The floating continuation also made the coverage
gate racy: the only coverage of the SubagentStart continuation's
no-context branch arm rode on an un-awaited .then, and on a loaded CI
runner the fork's per-file coverage snapshot beat it — master run
28798191671 failed the 100% branch gate on hooks-claude/src/index.ts at
99.03% (uncovered line 336) with the identical tree passing the PR run
three minutes earlier.

New shared primitive createDetachedRuns() in dsh-hook-protocol: a bridge
tracks each detached run chain, passes the tracker's abort signal to
runHook, and registers drain() as its effect disposer — drain aborts
still-running hook processes (a kill via the bash seam, not a wait out
to the 10-minute default hook timeout), then resolves once every chain
has settled. fiber.dispose() resolving now means the bridge's detached
work is quiescent (docs/defensive-patterns.md).

The subagent marker test disposes the bridge as its sync point, so the
formerly racy branch arm is executed deterministically before the file's
coverage snapshot; new tests pin abort-on-dispose promptness for both
bridges and the tracker's settle/drain contract in hook-protocol.

fb3f3a69ec fix: extend the schema cross-check to nested key paths; scope the catalog framing
Review findings on the config catalog:

The schema-subset check compared only top-level z.object keys against
top-level type members, so a nested loader-accepted key (agents[].id,
agentOptions.model, capabilities.*) missing from the declared type would
pass the gate unseen. The walk now collects nested object/array
compositions as key paths and resolves each against the declared config
type — through interfaces (heritage included), aliases, literals,
intersections, unions, arrays, indexed access, Partial-style wrappers, and
type references across package-local and workspace imports (re-export
chains included). The check stays presence-only and one-directional, and
only a definite miss is a violation: a path crossing a type the walk
cannot enumerate (an external package's) is skipped, never mis-reported.
The recursion guard applies at named declarations only — a structural
first child shares its span start with its parent, so a span-keyed guard
on every node mistakes ordinary descent for a cycle and silently turns
definite misses into unknowns.

The page and RFC framing also overstated the catalog as the exact
cordis.yml-settable surface: the paste is the plugin's full declared
config type, and a field the runtime schema deliberately excludes (the ACP
bridge's test-injected stream) is a runtime-only seam its own JSDoc marks.
Both now say so.

Five spec cases pin the new behavior: nested hidden key, workspace
intersection via star re-export, Partial wrapper, indexed-access
composition, and the external-type unknown path.

ea58bdf1e8 Merge remote-tracking branch 'origin/master' into worktree-dynamic-workflows
Master's reconstructable-requests overhaul (#179) meets the workflow tool:
- subagent-inprocess structured-output nudge becomes a system-prompt section
plus logged context (the injected-request waterfall shape is gone upstream)
- snapshot fixtures re-recorded on the merged tree so every request/header
carries the workflow tool; authored error-finish/cancel headers patched to

- architecture.md condensed back under its word ceiling; module graph regenerated

116e2c45a8 docs: flatten single-file catalog dirs to docs/tool-catalog.md and docs/persistence-catalog.md
A directory holding exactly one file adds a path level for nothing;
cordis-catalog/ keeps its folder because it holds two sibling pages
(events.md + services.md), matching config-catalog.md which was born flat.

docs/tool-catalog/tools.md -> docs/tool-catalog.md and
docs/persistence-catalog/log-events.md -> docs/persistence-catalog.md; their
generators' OUT paths and in-page relative links drop one directory level,
and every citation repo-wide (docs, RFCs, package READMEs, generator
headers) is updated. graph-atlas.md, config-catalog.md, and the doc graphs
regenerate with the new paths.

b0c7eadd23 feat: generated plugin config catalog (docs/config-catalog.md)
scripts/gen-config-catalog.ts walks every packages/<group>/<pkg> entry with
the TypeScript compiler API and emits docs/config-catalog.md: per loadable
plugin, the verbatim config declaration (JSDoc included) its apply/constructor
receives in a ts config-catalog fence, the inject requirements, resolved links
for every referenced type (package-local types pasted transitively, other
plugins' config types as intra-page anchors, LINK_MAP names to
core-data-structures, workspace types to source), and terse classification
lists for config-free plugins, abstract seams, and libraries — classification
is total, so a new package cannot go undocumented.

The walk enforces per-field JSDoc prose on every pasted declaration and
statically cross-checks the schemastery schema (z.object keys, z.intersect
composition across packages): every schema-validated key must be a declared
member of the config type. One violation existed repo-wide — the agents[].id
field in dsh-agent-loop — fixed by adding its JSDoc (which shifts the
cordis-catalog services page's source pointers; regenerated).

verify-config-catalog (--check) joins doc-sync; doc-typecheck learns the
ts config-catalog fence; gen-cordis-catalog exports its JSDoc/pointer helpers
and LINK_MAP for reuse. Negative-path spec in
packages/core/agent-core/tests/gen-config-catalog.spec.ts mirrors the
gen-cordis-catalog spec. Decision record:
docs/rfc/implemented/process/2026-07-06-generated-config-catalog.md (includes
the deliberate acceptance of README ## Config overlap).

9688870da3 docs(workflow): admit the node:vm escape concretely — absent globals are surface, not containment
The review's critical finding: the engine README and the tool
description read as if the missing filesystem/network/Node globals were
enforced, but a script can reach the host Function constructor via
globalThis.constructor.constructor and from it process and every Node
builtin. Per the trust premise this is ACCEPTED (model-written scripts,
bash-equivalent trust; genuine sandboxing is the deferred engine swap
already listed) — but the docs must say so instead of implying a wall.
The trust-premise sections (engine README, module doc, RFC) now name
the escape and its acceptance; the model-facing tool description says
the APIs are not PROVIDED rather than implying they are prevented.

3987425547 subagent-inprocess: stop the structured nudge loop once cancellation lands
A cancel that lands AFTER a child's clean turn end but before the nudge
continuation runs clears nothing — child.cancel() only kills queued or
running work — so the loop's turn-state check alone let a later
child.send() spend a fresh post-cancellation turn (and even capture a
structured result the caller had already abandoned). The loop condition
now also reads the run's own cancelled flag, re-evaluated after every
whenIdle(), through an accessor because closure assignments are
invisible to control-flow narrowing (an inline read lints always-true).
readResult keeps the outcome honest on this path: a completed-but-
uncaptured turn maps to aborted, not error, when a cancel is why the
nudging stopped — the cancel contract outranks the schema shortfall.

db4a39b024 workflow: close the review-found cancellation and child-lifecycle gaps
Three review findings on the engine's child seam, one mechanism each:

- Cancellation now bridges to run.cancel() on every in-flight child, not
just the shared request signal — the subagent seam leaves a provider
free to honor either channel, so the consumer drives both (listener
  removed in the child finally).
- A child result REJECTION (an infrastructure fault the seam allows) now
emits the paired workflow/agent-end before propagating, and propagates
as a fatal WorkflowError with the new AGENT_RESULT code — previously it
skipped agent-end (permanently open child for seq-matching observers)
and dissolved to a per-item null inside parallel()/pipeline(), letting
a broken provider read as an ordinary failed child. A rejection landing
after cancel stays a cancellation (cancelled outcome + CANCELLED).
- Every hook now guards its entry with a shared throwIfCancelled():
phase()/log() no longer emit observer events after a script caught an
earlier cancelled rejection, and parallel()/pipeline() refuse entry —
cancellation is the next HOOK boundary, not just the next agent().

e70227d581 workflow: pin the spec engine's concurrency — the auto default is one slot on small CI runners
The two tests that wait for two children IN FLIGHT (the pipeline
no-barrier test and the dropped-promise cancellation test) inherited the
engine's auto-resolved maxConcurrentAgents, which is
min(16, max(1, cores - 2)) — exactly 1 on the 2-core CI runner, so the
second child never started and vi.waitFor timed out. Reproduced locally
under taskset -c 0,1; the full spec passes there with the helper pinning
a fixed ceiling. Tests about the ceiling itself keep their explicit
overrides, and the auto-resolve arm stays covered by the
default-config provider tests.

39f2898502 fix review findings: last stale compact README line; stale-mark the Code Mode proposal
The compact-basic README's opening line still described summarization
as routed through the agent request pipeline — corrected to the direct
one-shot llm/stream contract the rest of the package documents. The
proposed Code Mode RFC gets a premise-stale note: it names the old
full-request agent/request seam, which now shapes call config only —
re-map onto the log channels and system-prompt/assemble before
implementing from it.

8bd80e5e9b fix review findings: one frozen seed through the waterfall; stale agent/request docs
Codex diff review, round 1, two (A) findings:

- The agent/request fallback resolved the RAW seed object — on later
steps the session's cached header fold — so a delegating listener
(await next(), mutate, return) could rewrite the fold in place and
the change would compare as already-baseline: no delta logged, the
persisted log unable to reconstruct the request (the dev invariant
would fire on the divergence, but the log would still lie). One
structuredClone'd, deep-frozen seed now serves both the listener
chain and the fallback — in-place shaping after delegation throws —
and Session.requestHeader() freezes its fold on update, so the leak
class is unrepresentable from either side. Pinned by a loop-level
  delegating-mutator test.
- Doc sweep for the old contract: agent README's event row (mutate
GenerateOptions / tool filtering → frozen config seed, replacement
out, logged header), compact-basic's module JSDoc (summarize routed
  through agent/request → direct one-shot at llm/stream), and
  architecture.md's event-domain line (request mutation → call-config
  shaping).

c58e5adee7 test: close the coverage gaps the gate found
surface.replaceGeneration gets its direct-access test (the getter folds
a pending delta itself, not only via a nodes read); the reconstruction
theorem drives maxTokens and stop through the config waterfall; the
compact-basic envelope drops a dead conditional — config.maxTokens is
required and validated positive, so this backend's envelope always
carries the cap (the return type's optionality exists for overriding
subclasses).

c0808d5126 docs: the governing principle — every LLM request is reconstructable from the session log
The reconstructability RFC is the principle's home: model-visible ⟺
logged in both forms, the mechanism (boundary derivation + header
fold), the enforcement (write-time round-trip guard, the dev
invariant), the corollaries ranked (prefix-cache stability first), the
MiniCode lineage with the provenance arrow inverted, and the
alternatives it beat — including the stateful transmission client
whose three-design archaeology lives in PR #162.

Placements per the one-home-per-fact taxonomy: a standing-order line in
root AGENTS.md (with displacement trims to stay inside the 1,575-word
ceiling), the principle statement in architecture.md § Session Log and
its Turn Flow lines (condensed to the ratcheted 1,630 ceiling), the
request-envelope section in core-data-structures/core.md with the
LlmCallConfig paste, both review-requested FIXMEs
(FIXME(call-config-shape) beside the type, FIXME(catalog-verbs) at the
catalog's drift-gate note), cookbook rows redirected off agent/request
(tool filtering → system-prompt/assemble, plan-mode prompt → sections/
inject()), and the llm/stream JSDoc stating the frozen-request
contract. RFC index and all generated catalogs regenerated.

7539231eca compact: the summary's provenance records its call envelope
compact/summary gains { model, maxTokens? } — the envelope the
summarize call actually used, reported by the backend that made the
call: summarize() now returns { summary, model, maxTokens? } instead of
bare blocks, so an overriding backend (template or remote summarizer)
reports its own envelope honestly and compactRegion logs it. 'Which
model wrote this summary' becomes answerable from the log alone, and
the one-shot summarize request — outside the loop's header-event fold
by design — is reconstructable from log + code (the reconstructability
RFC's scope statement).

a8986c2c8a Merge remote-tracking branch 'origin/master' into worktree-dynamic-workflows
Beyond the mechanical conflicts (provider capability lines vs master's new
inheritsParentContext field; generated catalogs regenerated rather than
hand-merged; knip/lockfile), three master-side reworks required semantic
adaptation of this branch:

- The persona rework removed AgentOptions.systemPrompt, which was the
structured-output instruction's channel. The instruction now rides the
SAME final-request enforcement listener that injects the schema'd tool:
appended per request to final.system (per-request wire state, not agent
prompt state). Tests assert the wire request (adapter.requests) instead
of child.options; the bare-direct-dispatch test pins the no-system arm.
- Tool guidance moved out of deployment prompts into per-tool prompt
sections; the examples' workflow paragraph became a tool:<toolName>
section contributed by dsh-tool-workflow (explicit-ask-only policy),
and both example personas resolve to master's minimal identity+behavior
form. tool-workflow gains inject: systemPrompt (+ peer dep, tsconfig
  ref); the export-shape guard updated.
- The uniform-RFC-format gate: the dynamic-workflows RFC restructured to
  the implemented/ skeleton (bare Status line; Proposal -> Decision;
What-was-rejected -> Alternatives considered; new Consequences), and
the overall-run-timeout deferral is now recorded in the RFC's Deferred
list. The doc-graphs atlas classification gains the workflows seam
  (workflow-vm implementation, tool-workflow consumer).

Master's harness-identity section made "empty assembled prompt" states
unreachable through the loop, so the instruction-append is a plain
undefined-ternary and the structured tests assert append-not-replace.
All snapshot goldens (including workflow-run) replay unchanged. Full
local CI-equivalent gate sequence green on the merged tree.

The dev-mode cross-check on llm/stream: a frozen request with a live
sessionId (the loop-built marker; hand-built one-shots stay unfrozen
and skipped) must carry messages deep-equal to the derivation over the
log prefix strictly before the in-flight step's step/start, and header
fields equal to the fold of the log's request/header* events. The
messages side rebuilds through a FRESH Session over the boundary
prefix — same projection code, zero shared state — so the live cache
cannot vouch for itself, and the seq-bounded rebuild is boundary-
correct: content appended after step/start (an agent/request-window
inject) legitimately belongs to the next request and cannot false-fire
the check. prepend:true only defends against the replay adapter's
short-circuit (append-registered); correctness never rests on listener
ordering. There is no divergence-allowance caveat: nothing can shape
request content outside the log.

2093a8898b loop: every request is built from the log — boundary snapshot, header events, config-only waterfall
The loop is now transmission-stateless; a request is a pure function of
(session log, this step's rendered assembly, current AgentOptions):

- The reconstruction boundary is step/start: the messages snapshot is
taken in the same synchronous frame immediately before the step/start
append, so the request's messages are exactly the derivation over
events[0..stepStartSeq) — an inject() from an agent/request listener
(or any concurrent task) lands after the boundary and joins the NEXT
request. This changes behavior for a synchronous step/start
session/event listener that appends content (master derived after the
append, so such a listener could reach the current request):
agent/pre-step is the sanctioned seam for current-request content.
- agent/request is re-typed to config-only: (agent, turn, step,
config: LlmCallConfig, next) → LlmCallConfig. The frozen seed comes
from AgentOptions on a loop instance's first request (explicit options
beat the logged baseline — fork overrides and resume reconfiguration
stay correct) and from the log's folded header afterwards; listeners
return a replacement to switch. Content shaping through the request is
no longer expressible — model-visible content flows through the log
  channels.
- recordRequestHeader appends whatever header event the request owes the
log before dispatch: an 'initial'/'resume' snapshot anchoring each
loop instance, a round-trip-verified delta on change, a 'fallback'
snapshot when the encoding cannot express it. Session.requestHeader()
  is the log's incrementally-folded baseline.
- Requests are deep-frozen before dispatch (deepFreeze exempts the
  AbortSignal — freezing one breaks AbortController.abort() outright);
frozen + sessionId is the loop-built marker the dev invariant keys on.

Ported from #162 and re-anchored on the log: the append-extension /
frozen-end-to-end / compaction-resend / prompt-change property tests,
plus new specs for the boundary semantics, resume anchoring, and the
end-to-end theorem (every recorded request rebuilds byte-equal from the
log alone). Live cache-hit e2e (request-cache.e2e.ts) verified against
the real DeepSeek API. Snapshot goldens intentionally stale until the
single re-record after the compact/summary envelope lands.

a4f7e757fa session: cache the derived history — one projection per node, frozen and shared
deriveMessages() no longer re-derives and re-clones the whole surface
per call: each node is projected exactly once, when first seen (O(new
nodes) per access), through the now-public deriveEventMessage — the
per-node pure function external reconstructors and the dev invariant
fold over a log prefix, so no two paths can disagree about a request's
messages. A surface rewrite (replace, invalidate) rebuilds, signalled
by SurfaceManager.replaceGeneration — monotonic, bumped by every folded
replace and by invalidate(), never reset. Callers get a fresh array
snapshot per call over SHARED deep-frozen messages: isolation of the
append-only log moves from per-call cloning to unrepresentability
(mutation throws), which the two isolation tests now pin directly.

a7b569850b session: the request header becomes logged state — request/header events + fold/diff/apply
Every conversation request's non-content half (system prompt, tool
schemas, call config — the EpochHeader) is now recorded in the session
log: a 'request/header' full snapshot (reason 'initial' | 'resume' |
'fallback') anchors the fold at conversation birth and process
boundaries, and 'request/header-delta' events (system line-trim,
name-keyed tools delta, whole config) encode mid-run changes. The pure
trio — foldRequestHeader / diffHeader / applyHeaderDelta — reconstructs
the header any request was built under from the log alone; the writer
contract round-trip-verifies every delta with a 'fallback' snapshot
when the encoding cannot express a change (pure tool reordering), so a
well-formed log always folds cleanly. Canonical absence: empty system
and empty tools normalize to absent fields, matching request builds.

Persistence and cordis catalogs regenerated; SessionEventMap paste and
EpochHeader added to the core-data-structures session page.

3f09159ab8 llm: LlmCallConfig + callConfigEquals + deepFreeze — the stateless request vocabulary
The call configuration (model + sampling scalars) becomes named
vocabulary: per-conversation state that the session log records as part
of the request header (the reconstructability RFC on this branch), with
callConfigEquals as the real-change detector behind logged header deltas
and deepFreeze as the ownership helper the loop applies to every built
request. dsh-llm stays stateless — request in, chunks out; no
conversation object lives here.

8510986909 compact: summarize is a direct one-shot llm/stream call, not a loop step
The summarization request no longer dispatches agent/request — that seam
shapes the loop's conversation requests; a hand-built one-shot's
interception surface is llm/stream, like every other direct call. The
model comes from summarizationModel falling back to the agent's own. The
turn/step parameters existed on the compact seam only to feed the
agent/request dispatch and leave compactIfNeeded/compactRegion.

Groundwork for making every conversation request a pure function of the
session log (reconstructability RFC, this branch): the seam split keeps
the loop's request path as the single thing the log must explain.

Ported from worktree-prompt-prefix-stability (PR #162) where it was
review-converged; catalog and producer/consumer graphs regenerated.

80250a8f2e workflow: drop the dead abandon-timer guard in drive()'s finally
The per-file branch gate caught it: drive()'s finally always cancels
first, and every first cancel() arms the abandon timer, so the
`!== undefined` guard's false arm was unreachable. clearTimeout
tolerates undefined by contract — call it unguarded.

706691a7df workflow: re-check cancellation after the slot acquire
Codex convergence round 1 on the review-response commits: agent()'s
post-acquire window was real and unguarded. `await acquireSlot()` yields
at least one microtask tick even when a slot is free (and a queued
waiter resumes a tick after its release), so a cancel() landing in that
tick let the continuation start a child carrying an ALREADY-aborted
signal — the in-code comment claimed the window could not exist. A
provider that subscribes only to future abort events (the test stub;
the seam does not promise pre-aborted-signal handling) would never
settle such a child, leaking it until the dispose grace abandoned the
run, and a backend that misses the pre-aborted signal would burn a full
model turn after the user cancelled.

agent() now re-checks isCancelled() immediately after the acquire
(inside the slot-owning try, so the finally still releases), making
every post-cancel path reject before subagents.start. New deterministic
regression: cancel() in the same synchronous frame as start() lands in
the free-slot await tick — the run settles cancelled with ZERO children
started (previously: one leaked child and a grace-delayed settle). The
raced-release test's comment now states what it actually pins (the
queued-waiter rejection path). Also aligns the RFC's auto-concurrency
formula with the code (min(16, max(1, availableParallelism() - 2))).

ed3972a9c6 workflow: linear meta-prefix scan (the regex backtracked exponentially)
Review finding, measured: the leading-trivia prefix regex
(`^\s*(?:comment|comment|\s+)*export …`) partitions a whitespace run
ambiguously between its outer `\s*` and the starred `\s+` alternative,
so a script that ultimately FAILS the match backtracks exponentially —
~19 ms at 35 leading whitespace characters, ~174 ms at 38, ×2.2 per
character; a realistic near-miss (a comment header, blank indented
lines, then `const meta` missing its `export`) did not finish in 10
seconds. The regex ran on the HOST stack inside the synchronous
`start()`, where no vm timeout applies and no abort can interleave — a
benign one-token typo, exactly what SCRIPT_PARSE exists to bounce back
to the model, hung the whole process instead of reaching that designed
recovery.

Replaced with a hand-rolled linear trivia scan (whitespace + `//` and
`/* */` comments — the module already scans characters for the literal)
followed by an anchored `^export\s+const\s+meta\s*=\s*` on the
remainder, whose quantifiers cannot backtrack ambiguously. An
unterminated block comment before the statement now gets its own
SCRIPT_PARSE message. Regressions: the near-miss shape must reject in
under a second (the old regex would trip the suite timeout), plus the
unterminated-leading-comment and comment-to-EOF edges.

2accf85714 workflow: simplify to the trust premise; settle result on cancellation
Two review responses that belong together — the same review argued the
engine was defending the wrong threat while a benign-input bug wedged
the product.

1) Drop hostile-value containment; state the trust premise.

Scripts are model-written — the same trust level as the model's bash
access — yet successive pre-push review rounds had ratcheted in defenses
that only matter against an adversarial author: trap-free proxy
rejection, accessor-never-invoked descriptor walks, realm-side
pre-rendering of thrown values, realm-built promises/arrays/error clones
with structural fatal recognition. That same author keeps a documented,
accepted, unkillable event-loop spin, so containing its error VALUES is
cost without a threat model — and the planned hardened engine
(worker/isolated-vm) gets value isolation by serialization and deletes
all of this machinery anyway.

What stays, because benign scripts hit it constantly: result never
rejects; dropped hook promises cannot become unhandled rejections; the
value boundary rejects LOUD everything JSON cannot carry (now a plain
recursive walk — getters are read ordinarily and their result is what
crosses; a throwing read fails loud); a "__proto__" key still copies as
a data property; the fatal-vs-null combinator discipline (now host
instanceof — unforgeable from the realm and simpler than clone-shape
recognition). What changes for scripts (documented in the engine
README): hooks hand back host values and host errors — in-script
`instanceof Error` on a hook failure is false (branch on e.name/e.code)
— and args are host-cloned once so a script cannot mutate the caller's
object. realm.ts drops 289 → 173 lines; the hostile-value test tables go
with it. The premise now leads the engine module doc, the README, and
the RFC's engine section, with the removed machinery recorded under
What was rejected.

2) result settles within the dispose grace of a cancellation.

Review finding (verified through the real registry + tool + engine): a
script parked on a promise no hook owns — `await new Promise(() => {})`,
`await Promise.race([])`, a returned never-settling thenable — could not
be settled by cancel(): hooks reject and children abort, but nothing
touches a promise the engine does not own, so `result` stayed pending
FOREVER (the previous cut even pinned that as intended). The tool awaits
run.result BEFORE its disposing finally, the registry awaits the tool,
the loop awaits the registry — one such script wedged the whole agent
turn past any abort, unrecoverable in-process; the mock engine in the
tool's abort test settles result on cancel, which is exactly the
behavior the real engine lacked, so no existing test could see it.

The seam contract now says it out loud: once a run is cancelled, result
SETTLES within the implementation's bounded grace even if the script
never does. The vm engine arms an abandon channel in cancel(); drive()
races the script against it, force-settling 'cancelled' at the grace
(the abandoned settlement stays contained; a post-slice synchronous spin
remains the documented limitation). dispose()'s outer race now exists
for child quiescence only, and `workflow/end` again fires exactly once
per started run. The old 'result stays pending' pin is FLIPPED to the
new contract (the pinned behavior was the bug); new regressions cover
cancel-then-settle on a parked script, a never-settling returned
thenable, and the full composition through the REAL registry + tool +
vm engine (tool-workflow gains workflow-vm/subagent devDeps for it).
agentsStarted JSDoc clarified while touching the vocabulary (accepted
calls, including ones still queued at cancellation).

3633cf90c8 fix(review): contain provider-removed listener failures; pin the model-via-request path
ds-review-bot round 2, both warnings:

- subagent/provider-removed now routes through emitLifecycle (per-listener
containment, the subagent/start|end precedent) instead of raw ctx.emit,
whose dispatch halts on the first throw: a throwing subscriber can no
longer starve a later mirror into keeping a stale tool, nor disrupt the
  backend fiber's teardown mid-disposer. provider-added deliberately keeps
propagation (register-time rollback semantics, like the system-prompt
registries); the asymmetry is documented on emitLifecycle, the event
  JSDoc, and the provider-lifecycle RFC.

- The documented model-via-agent/request fallback composes with a
{{model}} persona via the ownership rule itself: the plugin supplying
the model late states it early on the system-prompt/assemble waterfall.
Declined re-ordering render after agent/request — it would break the
agent/pre-step contract (compaction must measure the prompt the model
sees). New loop test pins the supply path end-to-end; the RFC's
{{model}} consequence bullet now covers supply as well as switch.

bffdee88e4 Merge origin/master: adopt the uniform RFC format
This branch's two RFCs (prompt-variables, provider-lifecycle events)
conform to the new contract in the same merge: 'Status: implemented'
dateless header, 'Rejected alternatives' -> 'Alternatives considered',
'What we give up' folded into '## Consequences' (per the uniform-format
RFC's own rejection of a costs-only closer), and the index regenerated
into the new INDEX.md. docs/rfc/README.md resolved to master's rewrite.

3f83a4ee96 review: the persona becomes the system-prompt plugin's deployment config
Review round 2 (tianyicui inline comments):

- dsh-system-prompt itself registers the harness:identity (-100) and
deployment:persona (0) sections — they must survive a swapped loop
plugin, so they leave dsh-agent-loop; the persona text is the plugin's
own validated 'persona' config. The model/cwd variables STAY on the
loop: runtime facts of the agents it drives.
- AgentOptions.systemPrompt is deleted with all its forwarding plumbing:
the app configs' systemPrompt keys become 'persona' routed through
dsh-agent-core (schema = z.intersect of the owners'), the ACP bridge
and tool-subagent stop carrying persona configuration, and subagent
children now render the deployment persona like every other agent.
- Example personas drop transport/interface trivia (ACP, CLI) — facts
  irrelevant to the model.
- Root CONTEXT.md removed (not idiomatic); its persona definition was
wrong under the new ownership anyway.
- Docs, READMEs, the prompt-variables RFC, and generated catalogs
updated; new loop test pins the assemble-waterfall escape valve
(an emptied assembly sends NO system field).

e6fad266a6 docs(rfc): define and enforce a uniform RFC format; adopt it across the corpus
Define the in-file RFC contract in docs/rfc/README.md § The file format:
the header block (`# RFC: <title>` plus a dateless Status enum
cross-checked against the lifecycle folder), the per-lifecycle body
skeleton (a Problem opener everywhere; Proposal/Alternatives considered/
Acceptance criteria/Risks in proposed/; present-tense Decision/
Consequences with proposal-era headings banned in implemented/; the
frozen proposal shape in rejected/), and a mandatory Alternatives
considered section with a date-fenced grandfather comment for pre-format
RFCs whose alternatives are not reconstructible from the record.

Enforce it with a new doc-sync gate, scripts/verify-rfc-format.ts, and
normalize all 112 RFCs to it: ~15 Status-line spellings collapse to the
enum, 29 Context openers become Problem, the 39 legacy-format XXX debt
markers are resolved and banned from reappearing, proposal-era sections
in implemented RFCs are rewritten to shipped reality (including the
web/fs/subagent seam RFCs' migration plans and test checklists, closing
the doc-tiers deferred-work item on the web seam), every RFC gains an
Alternatives considered section or the grandfather comment, and the
bilingual pair is re-mirrored and re-recorded.

Move the generated index tables out of README.md into a fully generated
docs/rfc/INDEX.md — gen-rfc-index now writes the whole file, and
verify-rfc-classification checks its freshness and rejects index-shaped
rows in the curated README — which makes room for the format contract to
live in the README front door instead of a separate FORMAT.md.

The decision record, and the first RFC written in the new format, is
docs/rfc/implemented/process/2026-07-05-uniform-rfc-format.md.

7234d41b91 workflow: hook promises and hook failures are realm-built too
Codex code-review round 5: agent()/parallel()/pipeline() returned HOST Promise
objects into the script realm — Object.getPrototypeOf(agent('x')) reached host
Promise.prototype, contradicting the realm contract (correctness containment,
not the accepted sandbox stance). The rejection channel had the same leak one
hop away: a caught hook failure was a host WorkflowError (host Error.prototype
chain), and phase()/log() threw host errors synchronously.

All three surfaces are realm-built now:
- hook promises: the realm's own Promise.resolve (bound at context setup)
assimilates the host promise, so the script-visible promise carries realm
prototypes; the realm promise gets the same no-op rejection consumer as the
host one (a script may drop it).
- hook failures: rejections and phase/log sync throws are translated at the
boundary into realm-built clones (name/code/message/fatal via an in-realm
factory); non-WorkflowError host failures become generic realm Errors
  carrying their describeThrown rendering.
- the combinators recognize FATAL clones structurally
(isFatalWorkflowErrorClone: proxy-guarded descriptor reads), preserving the
fatal-vs-null discipline across the boundary; a script forging the shape
kills only its own run. drive() maps any post-cancel failure to 'cancelled'
by run state (a CANCELLED clone deliberately fails the host instanceof).

Tests: realm-promise identity for all three hooks + host Promise.prototype
pollution unreachable; clone shape (instanceof realm Error, name/code/fatal/
message) with prototype-chain mutation staying realm-side; a rejecting
provider result crossing as a generic clone; phase/log sync-throw clones;
combinator catch branches (string throw, proxy throw, shape-miss forgery →
null; forged fatal → kills own run); existing fatal-propagation, cancellation,
and unhandled-rejection tests as canaries.
